• Defiler of Faith on the battlefield
• Phyrexian Altar on the battlefield
• Ondu Cleric on the battlefield
• Stoneforge Acolyte in hand

Steps

1. Cast Stoneforge Acolyte by paying
2. Defiler of Faith triggers, creating a 1/1 Soldier creature token
3. Whenever Stoneforge Acolyte enters the battlefield, it and Ondu Cleric trigger
4. Resolve the Stoneforge Acolyte trigger, putting a +1/+1 counter on it
5. Resolve the Ondu Cleric trigger, causing you to gain at least two life
6. Activate Phyrexian Altar by sacrificing the Soldier token, adding
7. Activate Sword of the Realms by paying , attaching it to Stoneforge Acolyte
8. Activate Phyrexian Altar by sacrificing Stoneforge Acolyte, adding
9. When Stoneforge Acolyte dies, Sword of the Realms triggers, returning Stoneforge Acolyte from your graveyard to your hand
10. Repeat
