• Plague of Vermin in hand
• Altar of Dementia on the battlefield
• Cauldron of Essence on the battlefield

Steps

1. Cast Plague of Vermin by paying and choosing to pay any amount of life, creating that many 1/1 Rat creature tokens
2. Activate Altar of Dementia by sacrificing a Rat
3. When the Rat dies, Cauldron of Essence triggers, causing each opponent to lose 1 life and you to gain 1 life
4. Resolve the Altar of Dementia ability
5. Repeat from step 2 as able
