• Legion Loyalty on the battlefield
• Jace, Wielder of Mysteries on the battlefield
• Mirror-Mad Phantasm does not have summoning sickness
• You have at least two opponents

Steps

1. Declare Mirror-Mad Phantasm as an attacker
2. Mirror-Mad Phantasm's myriad ability triggers, creating a token copy of itself tapped and attacking each other opponent
3. Activate a Mirror-Mad Phantasm token by paying , causing you to shuffle it into your library, then reveal and mill your entire library due to not having a card named Mirror-Mad Phantasm in it
4. Activate Jace's first loyalty ability by putting a loyalty counter on it, causing target player to mill two cards and you to win the game due to drawing from an empty library
