Prerequisites

• Blade of the Bloodchief on the battlefield
• Herd Baloth on the battlefield
• Altar of Dementia on the battlefield
• Blade of the Bloodchief attached to Herd Baloth
• You control at least one additional creature

Steps

1. Activate Altar of Dementia by sacrificing another creature
2. When the creature dies, Blade of the Bloodchief triggers, putting a +1/+1 counter on Herd Baloth
3. Herd Baloth triggers, creating a 4/4 Beast creature token
4. Resolve the Altar of Dementia ability, causing target player to mill cards equal to the sacrificed creature's power
5. Repeat
