• Altar of Dementia on the battlefield
• Necrosynthesis attached to Herd Baloth
• You control another creature to sacrifice

Steps

1. Activate Altar of Dementia by sacrificing a creature, milling target player equal to its power
2. Necrosynthesis triggers, putting a +1/+1 counter on Herd Baloth
3. Herd Baloth triggers, creating a 4/4 Beast creature token
4. Activate Altar of Dementia by sacrificing the Beast token, milling target player four
5. Repeat from step 2

• Infinite +1/+1 counters on Herd Baloth
• Infinite death triggers
• Infinite ETB
• Infinite LTB
