• Chainer, Dementia Master on the battlefield
• Wurmcoil Engine on the battlefield
• Phyrexian Altar on the battlefield
• Hinterland Sanctifier on the battlefield
• Your life total is at least 4

Steps

1. Activate Phyrexian Altar twice by sacrificing the Wurm tokens, adding
2. Activate Chainer by paying and 3 life, putting Wurmcoil Engine from your graveyard onto the battlefield
3. When Wurmcoil Engine enters the battlefield, Hinterland Sanctifier triggers, causing you to gain 1 life
4. Activate Phyrexian Altar by sacrificing Wurmcoil Engine, adding
5. When Wurmcoil Engine dies, it triggers, creating two 3/3 Wurm artifact creature tokens
6. When the Wurms enter the battlefield, Hinterland Sanctifier triggers twice, causing you to gain 2 life
7. Activate Phyrexian Altar twice by sacrificing the Wurms, adding
8. Repeat
